As a Case Manager with Proximity Learning Inc. (PLI), it is essential that you fully understand the responsibilities and expectations of your role. You will be asked to carefully review the Case Manager Job Description, and confirm that you have read and understood all aspects of the position before being placed as a Case Manager.

 

MEETING & COMPLIANCE REQUIREMENTS:  

The Case Manager ensures timely and compliant facilitation of all IEP-related meetings, including Annual, Revision, MDR, and Re-Evaluation IEPs (utilizing district-provided data when necessary). They participate in pre-meeting staffing and develop schedules to ensure adherence to due dates and legal timelines. As an integral member of the IEP team, they actively lead and contribute to the collaborative decision-making process.

 

IEP DEVELOPMENT & IMPLEMENTATION:  

Working closely with educators and families, the Case Manager develops and implements Individualized Education Programs in both virtual and in-person environments. They lead the creation of academic, behavioral, and transition goals, determine appropriate accommodations and modifications, and use pre- and post-assessment data to inform and refine IEPs.

 

MONITORING & DOCUMENTATION: 

The Case Manager consistently monitors student progress and provides virtual feedback and resources to ensure educational and legal compliance. They are responsible for maintaining accurate and up-to-date electronic academic records in accordance with district and PLI standards.

 

COORDINATION OF SERVICES:  

This role involves managing and coordinating special education services for students with a variety of learning needs, particularly in virtual learning environments. The Case Manager works with service providers to ensure that students’ physical, academic, behavioral, and emotional needs are fully supported.

 

COLLABORATION & RELATIONSHIPS: 

Effective communication and collaboration are central to the role. The Case Manager works closely with district teams to share data and maintain alignment. They also foster strong, supportive relationships with students, families, educators, and administrators to promote student success and a positive educational experience.
