Request to Drop Courses
When to fill out this form: You should fill out this form when you need to withdraw from a course due to workload, personal reasons, or changes in your teaching schedule.
Please note that we need at least a 2 weeks notice to drop a course in order to find a replacement for your class(es). You will be required to remain in the class, be present each day, and fulfill your teacher duties until a replacement is found.
HR must approve all drop requests. Classes cannot be dropped without HR approval.

When to fill out this form: You should complete this form when you decide to leave your position permanently, whether due to retirement, new job opportunities, or personal reasons.
Start Resignation - Please Note - We require a 2 weeks notice. If you are unable to provide a 2 weeks notice, you will not be eligible for rehire in the future.

Leave of Absence 24-25 HR Employee & Course Management
When to fill out this form: You should fill out this form when you need to take an extended leave from your duties, such as for medical reasons, family emergencies, or personal matters that require time away from work. Extended leave is more than 3 consecutive days.
Start Leave of Absence - Please note that a LOA is not a request for a time off request of less than 3 days. Please do not fill this out for anything that is less than 3 days and does not fall into one of the three categories below.
